This is the survey that we were using at CalVilla. There is some leftover space in the middle for a QR for an (eventual) online version of the survey and our email/social media.
Is there anything we can remove/simplify/add? I think this should be a work in progress and I’m happy to print a new version every time so there’s no rush to make it perfect.
